Guidelines for Those Having To Make Arrangements for a Deceased Loved One/ Family Member or Friend

  1. Cremation or Burial or Burial at Sea (ie. Disposing of remains).


  2. Service Options
    • A: - Memorial Service (Coffin not present at Church)
          - Normally proceeding to private cremation; or
          - Family in attendance at crematorium.
    • B: - Complete Service (Coffin present at Church)
          - Proceeding to private cremation; or
          - Family in attendance; or
          - Burial at grave site.
    • C: - Non-Attendance Private Cremation (Oakleigh Arrange: no family or friends in attendance at the Crematorium)
          - Possible Memorial Service before the Cremation; or
          - Possible Complete Service before the Cremation; or
          - no service at all.
    • D: - Requiem Mass (Coffin present at Church)
    • E: - Grave Side Service


  3. Viewing - Yes or No - If Yes either at the: Funeral Home; Church, Crematorium; or at the Grave Site (rare).


  4. Pall Bearers (only applicable for Complete Services) - Oakleigh Staff or Family nd Friends. Regarding the movement of coffin at Church/Crematorium/Burial he following must be decided:
    - Coffin taken in before mourners arrive and out after the service ends and mourners have left; or
    - Coffin taken in before mourners arrive and out during the service;
    or
    - Coffin taken in as service starts and out during the service (normal procedure for a Requiem Mass).


  5. Minister - Families own Minister or Oakleigh can arrange a Minster.


  6. Organist - Church or Oakleigh can arrange the Organist.


  7. Venue - Families own or Oakleigh can arrange.


  8. Press Notices - No press notices required by family or family to arrange their own or Oakleigh place notices on behalf of the family(ie. Death and funeral notice).


  9. Was the deceased wearing any jewellery - if Yes must this be left on or handed back to the family?


  10. Clothes - Leave on existing clothing or family to provide clothing to dress deceased in or coffin shroud to be used?


  11. Scattering of Ashes - see "Options Available Regarding Ashes"


  12. Death Certificate - will the family arrange same from Home Affairs or should Oakleigh obtain it and family collect from Oakleigh once ready?


  13. Personal Information - provide Oakleigh with the ID of the Deceased as well as that of the next-of-kin and information regarding the person/Executors (responsible for ccount).


  14. Method of Payment - Cash, credit card or cheque; account to Estate (subject to terms and conditions and Oakleigh approval) or Funeral policy.


  15. When making the funeral arrangements at Oakleigh, the Next-of-kin will be required to sign: - Oakleigh's Conditions of a Funeral;
    - Application for Cremation (if cremation);
    - Confirmation of Discussion and;
    - Pro-forma Account.


  16. Pace Maker - did the Deceased have a pace maker? Yes or No (if Yes and the deceased is to be cremation the pace maker must be removed).


  17. Occupational hazard - Did the deceased work in a hazardous area? (eg. Underground mining)


  18. Coffin Type - What coffin do you require? Coffins range from plain chip board with rope handles, plain pine, to Caskets.


  19. Floral Tribute - do you require fresh flowers on the coffin and in front of the church. Will the family arrange or should Oakleigh do so on the family's behalf?


  20. Programmes / Hymn Sheets - do you require hymn sheets for the Church Service (if applicable). Will the family arrange these or should Oakleigh do so on the family's behalf?


  21. Teas after Service - do you require Teas for after the service (if applicable)? Will the family arrange these or should Oakleigh do so on the family's behalf?
